Requirements for Lifeline Discount


Before you can get a Lifeline discount on your phone plan, you'll need to submit a Lifeline certification form as well as proof our your eligibility.


The Lifeline Certification is included during the on-line ordering process. If you order over the phone, you'll need to submit the certification after you get your new account number. Go here to submit a Lifeline certification form.


The Proof of Eligibility is any official document or program card that shows you are enrolled in the program that you are qualifying under (like SNAP card, or Section 8 award letter, etc) or, if you are qualifying based on your income, documents showing your income. You can see a list of acceptable proof documents here as well as methods for delivering that proof to us.
