Documentation Requirement for Lifeline Qualification

Acceptable forms of proof for program participation are:

• Screen Snapshot of your on-Line EBT program account (must show your name, account number and the account status. See instructions on the lower left of this page.)
• The current year's or prior year's statement of benefits from a qualifying state, federal, or Tribal program.
• A notice letter of participation in a qualifying state, federal, or Tribal program
• Program participation documentation. Just take a picture of your SNAP or EBT Card! (e.g. Supplemental Nutrition Assistance Program (SNAP) electronic benefit transfer card or Medicaid participation card (copy or scan of card))
• Other official documentation that demonstrates you or somebody in your household gets benefits from a qualifying state, federal, or Tribal assistance program

Acceptable forms of proof of household income are:

• The prior year's state, federal, or Tribal tax return
• Veterans Administration statement of benefits
• Retirement / Pension statement of benefits
• Three consecutive months worth of income statements from an employer or paycheck stubs within the current calendar year
• Social Security statement of benefits
• Federal or Tribal notice letter of participation in General Assistance
• Unemployment / Workman's Compensation statement of benefits
• Divorce decree, child support award, or other official financial document containing income information for at least three month's time.

*Document copies and pictures must be clear and legible, displaying your name and document or card number. Use hi-resolution on pictures.
