If you've already placed your order and have an account number, please include your account number with your
proof document.
There are two ways to qualify for Lifeline benefits. You can qualify for Lifeline based on
Program Participation or based on Income.
Program participation qualification is where you would qualify if you are currently
enrolled in a government assistance program.
Income qualification is where you qualify if your household income
is at or below 135% of the Federal Poverty Level. (150% in some states)
Proof documents are not retained by the service provider. Once they are reviewed, they will be destroyed.
If you have your EBT program card, just log into the online EBT account and take a screen capture to send us as proof of your lifeline eligibility.
To take a screen capture, navigate to your EBT account screen that shows your name, account number and account status. Then, hold down the ALT key and press "Print Screen". This will capture the screen to your computer's clipboard. Now, just "paste" into an email to us or into a Word Document and send that to us. To paste, just right click in the body of the email and click "paste" or, hold the SHIFT key and press "Insert".
