This free wireless plan is available to those qualified for Lifeline assistance. If you are in a state that does not have an eligibility database available, you will be required to provide proof of eligibility before we can send your phone. (see below for more information on proof documentation).
To get free wireless minutes which is made possible by the Lifeline program subsidy, you'll need to submit a Lifeline certification form as well as proof of your eligibility.
The Lifeline Certification form is included during the on-line ordering process. If you choose to place your order over the phone, you'll need to submit the certification after you get your new account number. Go here to submit a Lifeline certification form after placing a phone order.
Proof of Eligibility
Proof of Eligibility is any official document or program card that shows you are enrolled in the
program that you are qualifying under (like SNAP card, Medicaid card, or Section 8 award
letter, etc) or, if you are qualifying based on your income, documents showing your income. You
can see a list of acceptable proof documents here
as well as methods for delivering that proof to us.
If you have an EBT card, you can easily provide proof by sending us a screen capture of your on-line EBT account. You can find more information on getting a screen capture of your EBT account here.
Once you've completed the on-line order, follow the instructions for getting us a proof of eligibility so that we can send out your free handset.
